Vacant positions for which we are currently hiring:
Community Case Coordinator: Progressive human service agency seeks individual for full-time position to support professional care management staff and help aged consumers. Work is performed both in the office and at consumers’ homes. Duties include: assisting consumers to complete program applications; obtaining and verifying income documentation; processing reimbursement requests; coordinating services; and educating consumers/caregivers. This position will also interview individuals and make referrals for services.
Our ideal candidate has excellent communication, time management, and multi-tasking skills, along with the ability to effectively collaborate with others to ensure best service for our consumers. Individuals must be detail oriented and have good math and computer data entry skills.
Full Job Description

Minimum qualifications include:
- Post-secondary education and experience with completion of either an approved field placement or one year successful employment in a health care or human services setting; OR
- High school graduate and one year’s experience working with people from diverse backgrounds, preferably in a social services setting; OR
- Equivalent combinations of education and experience may be considered at the Agency’s sole discretion.
