Care Manager I

Want a career where you can make a difference?  Consider joining our team of top professionals.  This full-time position conducts assessments/reassessments of older consumers, coordinates and implements community based long term care services, provides ongoing monitoring, and serves on a protective services on-call rotation schedule.

Our ideal candidate has excellent communication skills, a desire to advocate for the needs of the elderly, and the ability to effectively collaborate with others to ensure best service.  Individuals must be detail oriented with excellent multi-tasking and organization skills.  Good computer data entry skills helpful.  In return, our salary is competitive and our benefits package is outstanding, with generous vacation and holidays, and regular Monday-Friday schedules.

A bachelor’s degree in social work/related social science and one year of successful employment in a human services setting is required.  Care management experience working with the elderly is preferred; however, other related bachelor-level education and training may be considered.

Casual Drivers

Our casual drivers generally work less than 15 hours per week, delivering meals to homebound elderly residents of Clearfield County. Casuals serve in a substitute role for our regular drivers.  Agency vans are used for all routes, which depart from & return to Clearfield.  Work hours are weekdays only, generally between 9:00AM – 3 PM, but may vary depending on assigned route.  Hourly wage of $7.60 paid. 

We are looking for honest, reliable people.  To succeed in this position, you must be willing and able to: drive in all weather conditions; lift 35 lbs; go up & down stairs; follow directions; and do simple math. Persons offered a position will be required to pass a physical, drug test, motor vehicle check & criminal history check. In addition, you must possess a valid Pennsylvania driver’s license and have a driving record that is acceptable to our auto insurer.
